- California law requires every incoming 1st grader must have a physical for 1st grade. Kindergarten teachers will send forms. Click here for a copy. Please turn it in to the health office when completed.
- Students in 6th grade. Don't forget to turn in your proof of TDAP by May to your current schools health office. Click here to see additional information.
